Helping Children Heal: Understanding TF-CBT and PCIT

As caregivers, it can be difficult to know how to best support a child who is struggling. Some children may have experienced a traumatic event that changes the way they think, feel, or interact with others. Others may be experiencing behavioral challenges that leave families feeling overwhelmed or unsure of what to do next. While every child’s experience is unique, effective, evidence-based treatments are available to help children heal, strengthen family relationships, and build skills that support long-term success.

At Foundations Counseling Center, we believe that every child deserves individualized care that meets them where they are. We are happy to now be able to serve children as young as the age of 2 by offering Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) and Parent-Child Interaction Therapy (PCIT). Although these therapies address different concerns, both are designed to help children thrive while equipping caregivers with practical tools to support their child’s growth.

When Trauma Impacts a Child
Children process difficult experiences differently than adults. After experiencing a traumatic event, some children may become withdrawn, anxious, fearful, or irritable, while others may have difficulty sleeping, concentrating, or regulating their emotions. Sometimes these changes appear immediately while other times they develop weeks or even months later.

Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) is an evidence-based psychotherapy designed for children and adolescents who have experienced trauma and are experiencing emotional or behavioral challenges as a result. During treatment, children and caregivers work together to build healthy coping skills, improve communication, and process difficult experiences in a safe and supportive therapeutic environment.

TF-CBT may be appropriate for children who have experienced events such as abuse, bullying, community violence, traumatic grief, serious accidents, natural disasters, or other traumatic experiences. Research has shown that TF-CBT can help reduce symptoms of post-traumatic stress, anxiety, depression, and other trauma-related concerns while strengthening caregiver support and resilience.

When Behavior Becomes a Challenge
Many young children test limits as they grow and learn, but persistent tantrums, difficulty following directions, or frequent behavioral struggles can sometimes leave caregivers feeling frustrated or unsure how to respond. These moments can place stress on the entire family, but they also present opportunities to strengthen the caregiver-child relationship.

Parent-Child Interaction Therapy (PCIT) is an evidence-based psychotherapy for children ages 2-7 experiencing emotional or behavioral concerns. One of the unique features of PCIT is live guidance where therapists work alongside caregivers during real-time interactions with their child. This allows families to practice new skills together while receiving immediate guidance and feedback.

Treatment takes place in two phases. The first, Child-Directed Interaction (CDI), focuses on strengthening the caregiver-child relationship through positive attention, praise, and play. The second, Parent Directed Interaction (PDI), teaches caregivers safe, consistent, and effective behavior management strategies that help children accept limits, follow directions, and develop positive behaviors. Together, these two phases help caregivers build confidence while creating lasting changes at home and in everyday life.
